WAIT ok so the empty basically feeds the people in it a dream to keep them asleep and to keep it quiet. We don't ever really see what that dream is like for Cas, we only see him awake in the empty. And Billie said that whoever reaped the brothers would drop them into the empty so they couldn't come back. What if the "heaven" that dean is in is really just a dream concocted by the empty to keep him asleep?
